A postoperative stricture of the common bile duct. Dr. Ram Chuttani from Boston demonstrated techniques for dilation of a postoperative stricture during the Course of Therapeutic Endoscopy at the Clinica Alemana in Santiago, Chile. Dr. Chuttani explained the reason for placing multiple stents in a stricture to maintain dilation, and then demonstrated the placement of stents within a stricture. The first maneuver is to perform a papillotomy to open the ampulla of Vater in order to gain access for stent placement. Dr. Chuttani then performed a controlled sphincterotomy. Following sphincterotomy the stricture was balloon dilated prior to stent placement. Dr. Chuttani plans to leave the stents in for six months before reevaluating the patient. This is a superb demonstration of stent placement across a tight postoperative stricture.
